CareTeam Hub was part of a connected health initiative. The goal was to build various applications within the Salesforce Healthcloud Platform that tied into the EHR. Each application was designed for specific clinicians. The application delivered information from the EHR that was relevant to each team and their specific needs. Ranging from Home dialysis Nurses, In Center Nurses, Social Workers, and Dietitians. I worked on the Tactical Application. The Tactical Application was a unique team working on improving clinic quality measures and patient care. In order to improve the quality metric scores of clinics it is directly related to improving patient compliance and treatment adherence.
Clinics received scores in various areas of care and then a cumulative risk score. The tactical team then came into the clinic and designed a strategic plan for the clinic to improve their metrics. This involved various touch points both clinic and patient related actions. This was custom work our developers had to build. However, we used as much native functionality as possible to cut on time and delivery costs.

We created a patient list with scores and highlighted focus areas so the tactical team could identify the patients that were directly effecting clinic scores. This allowed the tactical team to include specific patients as part of the goals and improve metrics for better outcomes.
Using the SalesForce HealthCloud Platform and API’s to our EHR we delivered targeted data to the tactical team that gave a deeper insight to the patients clinic scores and where interventions needed to happen in order for the patient to improve and have better treatment outcomes.
